You need a dog carrier and can keep him in the cabin, but prices and policies vary from airline to airline. Check their websites

It is possible to keep him with you in the cabin if he will fit.
You’ll need a pet carrier that is airline approved and can fit under the seat. Your dog must also have enough room to comfortably stand up and turn around in the carrier.
As far as I know, dogs cannot be carried on as baggage, they must have their own “ticket.” The cost depends on the airline. I know that if you put the dog in cargo, it’s about $150 - $200.
Your dog will also need a current health certificate from a state licenced vet (within 10 days of the flight). The vet has the forms, just call you vet and ask if this is a service they provide. If he is over 12 weeks old (I assume since he is so big), then he will also need a rabies certificate that was issued within the last 12 months (meaning, he needs to have proof that he got a rabies shot in the last year).
Other than that, if you put him in the cabin, you won’t need anything else. If you have to put him in the cargo area, you will need to label his kennel (not a soft-sided kennel) with your name and address, a live animal sticker, where he is going, etc.

Sarah is generally right, and as a backyard breeder/puppy mill, she probably ships a ton of puppies to ‘buyers’.
Short of it is:
Dog must be in an airline approved crate
Dog MUST remain in the kennel under the seat in front of you at ALL times
A fee must be paid to the airline for the dog. Anywhere from $75-$300.
While in the kennel, the dog must be able to COMFORTABLY stand and turn around. Any squishing of the dog and you may not be permitted to fly.
Dog needs to be UTD on vaccinations required for the state that you are flying to. Requirements vary by state.
Most airlines will NOT permit you to buy a seat for the dog. You can ask, but it will likely not be possible.
